Dear Member,
The purpose of this letter is to advise you that your birthday on July 21, will place you in a new age bracket, which will cause a change in your monthly Individual premium. The new premium of $341.00 is effective on Tuesday, August 01, 2006.
If you are a direct bill member, your new premium will be indicated on your monthly billing statement for the above-mentioned effective date. If your premium is automatically deducted, the new premium will automatically be deducted from your account.
If you have any questions, you may contact Health Plan of Nevada, Inc (HPN), Group Services.
